• Findings from your general practitioner or treating physician
• A letter of admission if you will be staying as an inpatient
• A referral from your treating surgeon if you are coming for outpatient treatment

Tuesdays: from 1:00pm onwards

By prior appointment arranged with the Chief Physician's secretary
